The Onboarding Breakpoint

When new hires hit the floor before they’re truly ready.

New hires are more than 4× as likely to experience a lost-time injury in their first month on the job. The breakpoint isn’t that you hired the wrong person. It’s that the system handed them the line before it could prove they could run it.

Diagnostic question: Which of your hires from the last 30 days are competent at their job, and which are still guessing?

The Shift Breakpoint

When the same SOP gets executed three different ways across three shifts.

First shift runs it the way it was written. Second shift learned it from first shift’s supervisor. By third shift, it has been improvised into something the SOP would not recognize. This is “shift drift,” and it follows a predictable slope: first shift good, second shift okay, third shift is where it comes apart. Audits do not fail because of bad documentation—they fail because the documentation stopped matching the floor three shifts ago.

Diagnostic question: If an auditor walked a critical line at 3 a.m. tomorrow, would they see the same process they’d see at 10 a.m. today?

The Growth Breakpoint

When the operation grew, but the knowledge holding it together didn’t grow with it.

A fourth shift. A second site. A new SKU, a new line. For years the operation ran on people who knew it cold and taught the next person over their shoulder. That tribal knowledge held the line together until growth stretched it past what it could carry. The veterans who kept the system in their heads get spread thin, or they retire and walk decades of know-how out the door. The temporary fixes invented to keep things moving quietly harden into the way things are done, and nobody wrote any of it down. The breakpoint shows up months after the growth, once the gaps finally reach the floor.

The Pressure Breakpoint

When training holds in normal conditions and breaks the moment the pressure hits.

The training completion report shows 100%. The dashboard says everyone is certified. Then orders get tight, a line goes down, someone calls out, and the shortcuts come back. Completion is not compliance. A record that says someone can do the task is not the same as watching them hold the standard when everything is going wrong at once.

Diagnostic question: When was the last time you watched an employee perform a critical task correctly under real pressure, versus saw a completion record that said they could?
